What are Steroids?

Steroid is the name of a broad family of compounds with similar chemical structures, not a single substance. The body itself produces steroid hormones like testosterone, estrogen, cortisol, and aldosterone from cholesterol. Therefore, saying "all steroids are the same" is incorrect.

A steroid injection mentioned in joint or pain treatment is often a corticosteroid injection. A steroid injection mentioned in bodybuilding is usually an injectable anabolic steroid. Although their names are similar, their purposes and risk profiles are not the same.

What are Anabolic Steroids?

Anabolic-androgenic steroids (AAS) are substances designed to enhance or mimic the effects of testosterone in the body. "Anabolic" refers to tissue building, especially muscle; "androgenic" refers to facial hair, voice deepening, and other male-type sexual characteristics.

Testosterone itself is an anabolic-androgenic steroid. Names like nandrolone, stanozolol, oxandrolone, methandrostenolone, and trenbolone are associated with this class. The DEA lists several of these substances among the steroids frequently encountered by U.S. law enforcement. This listing of names is not a recommendation for use; each has different effects, medical status, and risks. [2]

Medical use is not the same as non-medical use

In some cases, doctors may prescribe approved androgen medications for the treatment of diagnoses such as hypogonadism and in specific clinical situations. The goal here is to restore physiological levels based on laboratory analysis, symptoms, and medical history. High-dose, non-medical use for muscle and appearance, however, has a completely different risk-benefit balance.

TRT does not automatically mean a "steroid cycle"

Testosterone Replacement Therapy (TRT) is a treatment only when conducted under confirmed medical necessity and doctor's supervision. Using it to increase muscle in an individual with normal testosterone levels is not TRT. The presence of the same hormone does not equate the purpose of use and safety.

How do Anabolic Steroids Work in the Body?

AAS can alter gene expression by binding to androgen receptors in cells. In muscle tissue, this can help increase protein synthesis, nitrogen retention, and adaptation to exercise. However, this system does not only work in the biceps and chest: androgen receptors and hormonal signals also affect the reproductive system, brain, skin, cardiovascular system, and other tissues. Tablet, Gel, and Steroid Injection: Does the Form Eliminate Risk?

Anabolic steroids can come in forms such as oral tablets, injections, and topical gels. While the form may change absorption and the burden on some organs, no form makes non-medical AAS use risk-free.

Saying "I have no problem now" does not mean there is no future risk; changes in blood pressure, cholesterol, heart structure, and sperm production can remain asymptomatic for a long time.

AAS use has been associated with decreased HDL ("good") cholesterol, increased LDL, high blood pressure, blood thickening, thickening of the heart muscle, arrhythmias, and arterial stiffness. These can contribute to the risk of heart attack, stroke, heart failure, and sudden death. [7]

"Supplement" on the label does not prove it's not a drug

Miraculous, very rapid muscle growth; "same results as anabolic steroids"; "requires PCT"; products claiming to alter hormones and estrogen, but not providing full ingredients and independent testing, are particularly suspicious signals.

In Professional and Tested Sports

In WADA's 2026 Prohibited List, anabolic agents are listed under category S1, and anabolic-androgenic steroids are prohibited both in-competition and out-of-competition. According to the DEA, anabolic steroids and testosterone are Schedule III controlled substances in the U.S. federal system.
